Appointments to see Dr. Gorlowski can be made by calling the office at (814) 781-8677. Hours for appointments are Monday through Thursday from 8:00am to 3:30pm.

For each visit, please bring your insurance card and your co-pay. All co-pays must be paid on the day of the visit. Dr. Gorlowski’s office accepts most major medical plans. Co-pays can be made by paid by cash, check, MasterCard, Visa or Discover.

Dr. Gorlowski’s office has a 15 minute policy for your appointment. You will be given a 15 minute grace period to arrive for your appointment. After 15 minutes, you may be asked to reschedule at a later date.

In the event you need to cancel your appointment, kindly give a 24 hour notice. In the event that you are a no show without calling beforehand for your appointment, you will be charged $25.00 per incident. After three (3) no show appointments, you will be discharged from the office.

Quickly and conveniently obtain forms from our office. By filling out these forms ahead of time, you will save time during your visit. The forms will need to be signed at the bottom after printing. Please bring the completed and signed forms with you to your appointment.
